Community Update – Baseline Testing and Phase II Reopening

On June 29, The Glebe initiated COVID-19 baseline testing of all team members and residents in health care, assisted living and memory care as a prerequisite for beginning Phase II of the reopening processes for these higher levels of care.

The past few months have been tremendously challenging for everyone involved. Thank you for the ongoing support and patience as we work toward the goal of Phase II reopening for increased visitation and socialization of everyone in our community. To better understand where we are with the application for Phase II reopening of our community, the leadership team has compiled The Glebe’s baseline testing timeline and the requirement for Phase II as mandated by the Virginia Department of Health (VDH) per the guidelines set forth by the Centers for Medicare and Medicaid Services (CMS) and (CDC) Centers for Disease Control. 

The testing timeline is as follows:

June 29: The Glebe completed baseline testing for all health care, assisted living and memory care residents and team members. 
July 4: The Glebe received seven positive results for two residents, five team members and one self -reported independent living resident.
July 6: The Glebe completed the second round of baseline testing for all health care, assisted living and memory care residents and staff.
July 6-7: The Glebe tested 30 independent living residents due to possible contact tracing from the eight positive tests.
July 7: The Glebe received the first negative results for all eight previously positive cases.
July 07: The Glebe conducted the third COVID-19 test for all eight previously positive cases.
July 09-10: The Glebe received the second round of negative results for all eight previously positive cases.
July 10-13: The Glebe received the second round of baseline testing results for all health care, assisted living and memory care residents and staff.  All test results were negative.
July 11-13: The Glebe received negative results for the 30 independent living residents due to possible contract tracing from the eight positive tests.
July 13: The Glebe reduced isolation restrictions in assisted living and memory care based upon test results. Residents are able to leave their rooms and utilize common spaces with social distancing. Team members drop isolation precautions and resume using a surgical mask and hand hygiene.
July 13-14: The Glebe conducted the third round of baseline testing for health care residents and team members only.
July 14-19: The Glebe is waiting for the third baseline testing results for health care residents and team members only.

The Glebe is now closer than it has ever been to the initial reopening goal of expanded visitation for residents living in health care, assisted living and memory care. The leadership team is waiting to receive all negative test results for the third round of baseline testing in health care. Once the test results are returned, the team can apply for Phase II reopening as outlined by VDH per the guidelines set forth by the CMS and the CDC.

Phase II Requirements and Attestation per VDH, CMS and CDC:

  • City or county has been in Forward Virginia Phase II for at least 14 days
  • Nursing home has been in Reopening Phase I for at least 14 days
  • Baseline testing of all residents and all team members has been conducted; the predetermined plan for refusals was followed
  • Testing is occurring per recommendations in Section 4
  • No nursing home-onset cases in residents for the past 14 days
  • No new cases in team members for the past 14 days
  • Community is reporting to the NHSN LTCF COVID-19 Module weekly
  • The community has adequate staffing as reported in NHSN
  • The community has access to adequate PPE as reported in NHSN
  • The community has access to adequate testing as reported in NHSN
  • Infection prevention and control assessment (IPCA) has been completed
  • IPC measures have been maintained or implemented based on the assessment
  • Phase II Attestation has been sent to the LHD

Phase II Nursing Home Allowances per VDH, CMS and CDC:

Visitation

Visitation is generally prohibited, except for compassionate care situations. In those limited situations, visitors are screened, and additional precautions are taken, including social distancing and hand hygiene (e.g., use alcohol-based hand rub upon entry). All visitors must wear a cloth face covering or facemask for the duration of their visitTypes of visitation may continue under limited controlled conditions coordinated by the nursing home in consideration of social distancing and universal source control (e.g., window visits, outside visits). Limited visitation is at the discretion of each nursing home based on their temporary visitation policy and capacity for implementation.

Dining

Communal dining is limited to COVID-19 negative or asymptomatic residents or residents who meet the criteria for discontinuation of transmission-based precautions. Residents may eat in the same room with social distancing (limited number of people at tables and spaced by at least six feet).

Social and Group Activities/Outings

Group activities, including outings, limited (for asymptomatic or COVID-19 negative residents or residents who meet criteria for discontinuation of transmission-based precautions) with no more than 10 people and social distancing among residents, appropriate hand hygiene and use of a cloth face covering or facemask.

Resident trips outside the facility for non-medical necessary reason

Might be allowed to asymptomatic or COVID-19 negative residents or residents who meet criteria for discontinuation of transmission-based precautions, only in addition to social distancing, appropriate hand hygiene and use of a cloth face covering or facemask.

Salon

Entry of the beautician or barber is generally prohibited but if a facility deems necessary to allow entry of a beautician or barber, follow salon guidance.  Facility staff are screened and will be allowed to enter for services.

Fitness/Gym

Gym access is limited to COVID-19 negative or asymptomatic residents or residents who meet criteria for discontinuation of transmission-based precautions, but residents may use gym equipment with social distancing (limited number of people in the gym and spaced by at least 10 feet), hand hygiene and cloth face covering or facemask. Gym equipment should be properly disinfected between each resident.

Forward Virginia Guidance to Nursing Homes

A nursing home’s reopening should lag the general community’s reopening by 14 days. A Local Health District (LHD) might recommend the facility delay progressing to the next phase if a substantial increase in community transmission occurs. The LHD would communicate this with the facility with rationale and provide a threshold regarding when the facility would be eligible to progress.

Screening Staff/Residents/Visitors

As recommended by CMS, everyone who enters The Glebe will continue to be screened daily. Each team member has been educated regarding the process of self-reporting which includes travel history and signs/symptoms of respiratory illness. The leadership team asks residents to self- monitor and report any signs/symptoms immediately to the clinic nurse by calling 591-2198.

Independent Living Residents and Phase II

Family Member Visitation-July 20th:

Beginning Monday, July 20, family members may visit residents in their independent living apartment/cottage. Please note, all family members will be asked to visit in the apartment or cottage, refraining from visiting in community common spaces. All family members are required to wear a face mask during their visit and limit no more than two family members in the apartment/cottage at one time. 

Resident Face Mask Reminder:

Under the Governor’s executive order, any person age ten and older must always wear a mask or face covering while entering, exiting, traveling through and spending time in public settings. All residents and staff will be required to wear a face mask whenever they are in the community common spaces. 

Resident Use of Community Common Space:

Residents are moving about the greater community and common space more and more each day.  As a reminder; please wear a face mask and carry hand sanitizer whenever possible. 

  • Practice ongoing hand hygiene and limiting touch of public surface areas.
  • Please continue to practice social distancing, 6’ or more from a neighbor or staff member.
  • No group gathering.
  • Follow social distancing signage throughout the community.

Housekeeping Services/Facilities Work Orders

Housekeeping and facilities services will continue with regular cleaning and work order requests for apartments and cottages. Housekeepers and facilities staff will wear PPE when entering the apartment/cottage.  Residents may stay in their apartment/cottage during housekeeping and maintenance services. Please practice social distancing and if possible, utilize an alternate room in the apartment/cottage during the service period. Dining Services

Meal Delivery:

Dining services will continue to provide meal delivery/offerings (3) times per day.

Allegheny Dining Room:

The Dining Room will be open from 4:00-6:00 Monday – Saturday, dinner service only. Seating will be expanded into the Botetourt Room to allow appropriate social distancing. Sunday will offer a mid-day meal, hours to be determined. You may begin making reservations by calling Julie McNutt, 591- 2163.  *Dining Services will provide specific details outlining hours, reservations, and menus next week.

Groceries/Grab and Go:

The Depot Café will open for lunch services beginning Monday, July 20, between 11:00 a.m. – 1:00 p.m. Reservations are required and no more than 10 residents are permitted in the café during this time. *Please call Julie McNutt, 591-2163 for reservations. 

Continue to call the Depot Café for grocery and grab and go food items.

Farmers Market Produce Stand: Beginning Friday, July 17

Dining services and resident services will continue to offer a weekly produce stand at the main entrance. The produce stand is open between the hours of 10:00-11:30 a.m. Please follow the directional signs for entering and exiting, utilize the social distancing floor signs. The number of residents will be monitored to limit to six residents in line at one time.  Please wear a face mask. Team members will place vegetables in a plastic bag and direct shoppers toward the exit. 

Salon

Please call the salon to schedule an appointment. The salon will be limited to no more than two residents at a time. The salon team will call each resident 15 minutes prior to their scheduled appointment so they can sanitize the workstations prior to arrival. The salon will be secured to avoid any gathering in the waiting room area.  he salon will offer appointments Monday – Fridays, 9 a.m. – 4 p.m.  Please call, 591-2166 to schedule an appointment.

Transportation

Transportation will continue to offer transport services for necessary medical appointments.

Wellness

Fitness Equipment:

The 2nd floor fitness room will reopen beginning, Monday, July 20 between 8 a.m. – 3:30 p.m. The number of residents using the equipment will be limited to no more than five at a time. Each resident will be required to utilize a piece of equipment that is no less than 10 feet from another resident. Residents are requested to reserve a time by calling the Wellness staff at 591-2122. Wellness will provide an attendant in the exercise room to ensure the sanitizing of equipment and social distancing.

Exercise Classes:

The 3rd floor group fitness classes will resume beginning the week of July 20. More information will be distributed next week.

Chair Yoga:

Chair Yoga will be offered Monday, Wednesday, and Friday at 9 a.m. and 10 a.m. Six residents will be allowed to participate in each class. Please contact Wellness at 591-2152, to reserve a chair.

1:1 Fitness Prescription Appointments:

Victor and Stephanie will begin offering a 1:1 fitness prescription appointment in the fitness center. Please contact wellness to schedule an appointment.  591-2122.

Pool:

Beginning, Monday, July 20, the pool will be open between 8 a.m. – 12 p.m.  Four residents can be accommodated at a time. Please contact the Wellness team for reservations, 591-2122. *More details to come next week.
